Insurance

We will gladly file any dental claim forms for you, as a courtesy. Dr Rosenfeld is a non-participating provider and therefore he is considered out-of-network. You must provide your insurance card with full information at your initial appointment. Verification of benefits will be completed prior to the beginning of the appointment. During this visit, you will be required to pay a minimum of ½ the standard fee for the procedure performed. Should any treatment have an unpaid balance as a result of less insurance coverage than expected, you will be responsible for the difference. Dr. Mann and Dr. Patel participate with some dental plans. Please speak to Jeanette about your coverage and review the expenses that you will be responsible for.
